This is an intermediate session. We recommend students have a fundamental grasp of RCA techniques prior to taking this workshop.

Who Should Attend?
Individuals who manage, lead, or participate in problem-solving teams, corrective and preventive action activities, or are responsible for customer complaint issues.

Our comprehensive two-day RCA session provides:

  • Fundamental understanding of the RCA
  • Tools for use during the RCA process
  • Workshops to effectively document RCA results 

Students Will:

  • Learn and understand the 8-D model
  • Introduce the "5 Whys" technique as a tool to conduct RCA
  • Use "Cause and Effect Diagrams" as a tool in problem solving
  • Identify how team problem solving benefits your organization
  • Learn how "Structured Brainstorming" enhances the "Team Approach"
  • Learn how to document non-conformances, trigger effective action, and document action taken

Brad Ostash is a Business Management Consultant, ISO9000 and AS9100 Assessor, and Management System Consultant.

For over 35 years, Ostash has helped manufacturing organizations improve their production lines. As an experienced Quality Assurance Engineer in the manufacturing and service sectors, Ostash has audited, registered, and trained companies on ISO9000 and AS9100 standards.
